Testosterone & Women

Now, whilst this study was looking at premenopausal women, it still holds value. This is because testosterone in women has not been nearly as well studied (like man other areas of women's health), but also because these help indicate ranges - specifically what optimal ranges - of testosterone for women might be - something that can be helpful for women in peri or menopause to help better assess their needs and potential BHRT use.

This study aimed to establish normal reference ranges for testosterone levels in healthy, premenopausal women with regular menstrual cycles. Researchers measured total, free, and bioavailable testosterone, along with sex hormone-binding globulin (SHBG), in 161 women aged 18–49. Blood samples were collected at different phases of the menstrual cycle (follicular, mid-cycle, and luteal), but the small hormonal fluctuations across phases meant that the reference ranges can apply regardless of when the sample is taken.

Key findings:

  • Testosterone levels declined with age, while SHBG stayed relatively stable.
  • For a typical 30-year-old woman, the reference ranges (5th to 95th percentile) were:
    • Total testosterone: 15–46 ng/dL
    • Free testosterone: 1.2–6.4 pg/mL
    • Calculated free testosterone: 1.3–5.6 pg/mL
    • Bioavailable testosterone: 1.12–7.62 ng/dL
    • SHBG: 18–86 nmol/L

These reference ranges help provide a clearer standard for evaluating testosterone levels in premenopausal women, which can support better diagnosis and treatment of hormonal imbalances.
